About these forms

These forms are provided by Labflow, a Magentus company so you can contact us, register interest or give feedback. Using a form doesn’t create a contract to supply anything — any products or services are covered by a separate agreement.

What you agree to

• The details you give are accurate and are yours to share (or you have permission to share them).
• You won’t include patient-identifiable or health information about anyone.
• You won’t misuse the forms — for example by sending spam, malicious content or automated submissions, or by trying to access data that isn’t yours.

Your information

How we handle what you send is explained in our Privacy notice, including that answers are saved as you type.

Availability

We work to keep the forms available and accurate, but they are provided as they are and may occasionally be unavailable. If something goes wrong, you can also reach us through labflow.ai.

Our content

The Labflow name, logo and the content of these pages belong to Labflow. Please don’t reuse them without permission.

Liability

To the extent the law allows, we aren’t liable for indirect loss arising from using these forms. Nothing in these terms limits rights you have under consumer law that can’t be excluded.
